TeamKID Registration

We’re excited that your child is joining TeamKID at Brooker Baptist Church!

TeamKID is a fun, Christ-centered Wednesday night program for children 4 years old through 6th grade. Each week, kids enjoy exciting games, engaging Bible lessons, Scripture memory, and friendships while growing in their relationship with Jesus.

Please complete the registration form below. One registration form is required for each child attending TeamKID.

TeamKID meets Wednesdays from 6:00–8:00 PM.

TeamKID Liability & Medical Release

I, the undersigned parent/legal guardian, give permission for my child(ren) to participate in “TeamKID” activities hosted by Brooker Baptist Church, including weekly on-campus activities, games, special events, transportation (if applicable), and any approved off-site activities.

I understand that every reasonable precaution will be taken to ensure the safety and well-being of all participants. However, I acknowledge that participation in church activities may involve certain risks, including but not limited to accidents, injuries, illness, transportation-related incidents, and other unforeseen events.

In consideration of my child’s participation, I voluntarily assume all risks and agree to release, waive, discharge, and hold harmless Brooker Baptist Church, its pastors, staff, volunteers, drivers, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, illness, or damage that may occur during TeamKID activities, except in cases of gross negligence or intentional misconduct.

In the event of a medical emergency, I authorize Brooker Baptist Church staff or volunteers to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am financially responsible for any medical expenses incurred.

I also grant permission for photographs and/or video taken during TeamKID activities to be used by Brooker Baptist Church for ministry, promotional, website, and social media purposes unless I notify the church in writing.

By signing below, I acknowledge that I have read and understand this release and voluntarily agree to its terms.
